Assessment, aims and limitations
Clinical findings may lead to treatment, postponement, a different option or no procedure. Infection, skin disease, bleeding risk, uncontrolled health conditions, previous surgery or expectations that cannot be met safely need careful review. No procedure can guarantee a particular sexual function, size change, sensation, confidence or appearance.
Options differ materially. Filler is an injection-based, temporary intervention; penile prostheses are permanent surgical devices with different malleable and inflatable systems; labiaplasty reshapes selected labial tissue and should be planned conservatively. Anaesthetic, facility, monitoring and observation arrangements depend on the individual plan and must be stated in writing.
Travellers should allow time for assessment, the procedure, discharge and any planned review before flying home. Recovery can include swelling, bruising, tenderness, altered sensitivity and activity restrictions. Follow the treating team's wound-care and medication instructions. Worsening pain, fever, spreading redness, discharge, wound opening, heavy bleeding or difficulty passing urine requires prompt clinical assessment.
Questions from you
These notes answer questions people raise before they accept a written treatment plan. They describe what that plan should make clear. They are not a promise that any partner hospital uses a particular technique, device or payment arrangement. With your consent, an enquiry may be shared with an authorised healthcare partner.
How can a penile prosthesis affect erection, orgasm, ejaculation and sensation?
A prosthesis provides an erection when other treatment for erectile dysfunction has not been enough. It does not restore a lost orgasm or ejaculation, and it is not a cosmetic lengthening device. Source: EAU guidance on erectile dysfunction.
What is the erectile-dysfunction diagnosis, which other treatments have been tried, and why a malleable or a three-piece implant?
EAU guidance considers a prosthesis when other treatments for erectile dysfunction have not been enough or are unsuitable. The result of tablets, a vacuum device or injections should be written first. A malleable rod stays semi-rigid and has fewer mechanical parts. A three-piece inflatable prosthesis looks more natural when it is not inflated. The pump, reservoir and mechanical failure are separate reasons for revision. Neither device is implanted to lengthen the penis cosmetically. Source: EAU guidance on erectile dysfunction.
What is the exact implant brand and model, what does any warranty cover, and what is the plan for infection or mechanical failure?
The written plan should name the brand and model and the record that will be given to you. A manufacturer’s warranty does not automatically pay for infection treatment, removal surgery or another journey. An infected prosthesis is usually removed. Replacing it in the same sitting is not guaranteed. Mechanical failure means a separate revision operation. Source: EAU guidance on erectile dysfunction.
If bleeding, infection or a device problem appears after I go home, who follows it and which cost is whose?
An infected prosthesis needs review without delay. Removal is a separate operation, and a manufacturer warranty does not automatically pay for it or for another journey. The NHS asks people treated abroad to get the cost of further surgery and a return journey in writing beforehand. Travel insurance usually does not cover surgical complications. This page does not say that any partner pays to remove the device. Sources: EAU guidance on erectile dysfunction, NHS advice on cosmetic surgery abroad.
Can a three-piece inflatable penile implant be seen or felt when it is deflated?
The cylinders and reservoir are placed inside the body, so the implant is not an external device; when deflated, the penis is intended to look more relaxed than with a semi-rigid implant. The pump sits in the scrotum and may be felt there. How noticeable any part is varies by person, device and healing, so it is not possible to promise that a partner will never see or feel it. Ask the urologist to show you the proposed model and explain where each component sits and what to expect once healing is complete. BAUS patient guidance describes its internal components and scrotal pump.
Does using a three-piece penile implant require hand strength or dexterity?
A three-piece inflatable implant is operated by using a pump placed in the scrotum to move fluid into the penile cylinders, then activating its release mechanism to deflate it. This involves handling a small control, so hand strength, sensation and coordination are reasonable topics to discuss before choosing a device. BAUS notes that malleable implants may be useful for people with poor dexterity, but that does not make one type universally preferable. Ask the urologist to demonstrate the proposed pump and assess whether you can use it comfortably. BAUS patient guidance discusses dexterity in device selection.
How long does a three-piece penile implant last, and what happens if it fails?
There is no fixed lifespan for an individual implant. EAU guideline data summarise device survival of about 87% at five years and 77% at ten years across studies, but these are group estimates that vary by device, follow-up and patient factors. Mechanical failure can mean that the implant no longer works as intended and may require assessment and revision surgery; infection is a separate problem that can require removal. Ask which model is proposed, what its warranty covers, and who would arrange and fund assessment or revision if you are abroad. A warranty is not a guarantee of lifetime function or complication cover. EAU guidance reports group survival estimates and infection-management context.
Will I lose penile length after a three-piece inflatable implant?
A three-piece inflatable implant provides rigidity for erectile dysfunction; it is not designed to lengthen the penis or guarantee restoration of a previous measurement. BAUS guidance says implants do not restore length lost through prolonged erectile dysfunction or Peyronie’s disease. It cannot predict an individual’s result or establish that everyone will lose length. Ask the urologist how current dimensions and expectations will be assessed and documented before surgery. BAUS patient guidance explains the limits of what an implant can do.
What happens to my natural erections after a three-piece penile implant?
A three-piece implant creates rigidity mechanically when it is inflated; it does not restore the body’s natural erection mechanism. It is generally considered for erectile dysfunction when other treatments have not worked or are unsuitable. Whether any spontaneous erectile function remains cannot be predicted from the device type alone, because the underlying cause and individual health matter. Ask the urologist what the implant is intended to provide, how to operate it and what to expect from erections without inflation before consenting. BAUS patient guidance explains the purpose of the implant and its two operating types.
