What is a DHI hair transplant?

DHI means direct hair implantation. In the defined plan on this page, individual follicular grafts are harvested from the donor area using FUE and placed into thinning or balding scalp areas with a DHI or Choi implanter. FUE describes how the grafts are taken; DHI describes the implantation approach. The surgeon still has to diagnose the cause of hair loss and design the hairline, density and recipient areas.

A hair transplant redistributes existing follicles. It cannot create new follicles or guarantee a particular density, hairline or final appearance. Who might consider DHI?

A transplant may be considered by people with a diagnosis of permanent pattern hair loss and enough suitable donor hair for the proposed coverage. The consultation should examine the pattern and progression of loss, donor density, hair texture, contrast between hair and skin, the size of the recipient area, previous procedures, general health, medicines, smoking and expectations.

Not every type of hair loss is suitable for transplantation. Hair loss with a different cause, such as patchy alopecia areata, needs appropriate medical assessment before a cosmetic transplant is considered. If hair loss is still progressing, the design should account for possible future thinning around the transplanted area rather than treating one photograph as the whole long-term plan.

The graft number is personal and should follow the donor examination and the agreed recipient areas. A “high graft count” or unlimited graft promise is not part of this session guide, and more grafts are not automatically safer or more suitable.

How is FUE and DHI planned?

• Diagnosis and design: the clinician reviews the cause of hair loss, maps the donor and recipient areas and agrees a realistic hairline and coverage goal.
• FUE harvesting: individual follicular grafts are removed from a suitable hair-bearing donor area rather than taking a strip of scalp.
• Graft preparation: the harvested grafts are handled and prepared for placement, with attention to their follicular structure and the planned recipient area.
• DHI implantation: grafts are loaded into an implanter pen and placed in selected scalp positions. Direction, angle and distribution are clinical decisions, not a standard number for every patient.

The treatment plan may differ for the hairline, temples, mid-scalp or crown. A second session, an additional body area or a separate procedure should be assessed and itemised rather than assumed to be included in one DHI session.

Session time, anaesthesia and stay in Turkey

Hair transplant treatment usually takes a full day. It is generally performed with local anaesthesia, sometimes with sedation, so the scalp can be numbed while you remain under clinical observation. The exact time depends on the number and type of grafts, the donor and recipient areas, the team and the agreed technique.

An overnight hospital stay is not usually needed for a standard hair transplant, but the discharge plan depends on the facility, your health and the procedure. Arrange transport or an accompanying adult if advised after sedation. The written quote should identify the clinician, extraction and implantation scope, anaesthesia, facility, dressings or spray, prescribed medication, follow-up and any hotel or transfer arrangements.

Planning your hair-transplant stay in Istanbul

Plan around the consultation, hairline design, the session, discharge instructions and an early review if recommended. Keep your return travel and schedule flexible. You should not plan strenuous exercise, a crowded sightseeing programme or activities that rub the grafts during the early recovery period.

Bring your medical history, medication and supplement list, allergy information and details of previous hair treatments. Ask how to sleep, wash the hair, protect the recipient area and contact the team from home. Flights, extra nights, travel insurance and a companion’s costs are separate unless expressly included in writing.

What may be included in the written package?

Ask for each service in writing rather than relying on “all-inclusive”. Depending on the agreed plan, the quote may identify:

• the consultation, diagnosis and personalised donor/recipient design;
• one FUE harvesting and DHI/Choi implantation session with the agreed graft plan;
• the responsible clinician, anaesthesia, facility and clinical team;
• dressings or spray, prescribed medication and early aftercare instructions;
• planned checks, follow-up contact and coordination support;
• airport transfers and hotel nights if listed; and
• the confirmation that unlimited grafts, a second session, automatic PRP and long-term medication are not included.

What may be outside the price?

• return flights, travel insurance, meals and personal spending;
• a companion’s travel, accommodation or other expenses;
• a second session, unlimited grafts or treatment of another recipient area;
• an automatic PRP series, long-term medication or additional treatments;
• extra hotel nights, special tests, extended local care or unexpected facility costs; and
• treatment for complications outside the written agreement or after you return home.

Preparing for a DHI hair transplant

• Share the history and pattern of your hair loss, previous procedures, medical conditions, allergies, medicines, supplements and smoking or nicotine use.
• Ask whether the diagnosis requires medical treatment or a period of observation before transplantation, especially if the loss is patchy or changing quickly.
• Confirm the planned donor and recipient areas, hairline design, estimated graft range and whether shaving or a particular haircut is required.
• Follow the clinician’s instructions about washing, alcohol, smoking, fasting and medicines. Do not stop prescribed treatment without medical advice.
• Arrange transport or an accompanying adult if sedation is used, and prepare a clean, comfortable place to sleep with your head protected as instructed.
• Confirm how to use the spray or medication, when gentle washing begins, how to protect the grafts and how follow-up works after you return home.

Recovery, growth and long-term follow-up

The scalp may feel tight, sore or swollen for a few days. Temporary scabbing can appear in the recipient area, and small scars may be visible in the donor area after FUE. Your surgeon will tell you how to use the spray, wash the scalp and avoid touching or rubbing the grafts. The transplanted grafts are not secure during the first two weeks, so protect them carefully.

You may need around 1–2 weeks off work, and strenuous exercise may need to be reduced during the first month. Transplanted hair commonly sheds after a few weeks before new growth begins. New hair often starts to appear after around four months, while the full result may take 10–18 months. These are general milestones, not a guarantee or a substitute for your clinician’s review.

Hair loss can continue around the transplant. Long-term planning may therefore include monitoring or medical treatment, but long-term medication is outside this price guide unless expressly written into a separate plan. A transplant does not stop every future change in the untreated hair.

Possible problems include bleeding, infection, anaesthetic reaction, swelling, pain, scarring, temporary numbness, inflammation around a follicle, grafts not growing as expected, noticeable scarring and ongoing thinning. Contact the surgical team promptly for severe or worsening pain, heavy bleeding, fever, spreading redness, pus, marked swelling or another unexpected symptom.
