ESG — endoscopic sleeve gastroplasty — folds the stomach from the inside with stitches passed through an endoscope. No abdominal cut, no staple line, and no part of the stomach is taken out. The tube is held by those sutures; it is not a laparoscopic sleeve gastrectomy, not a gastric bypass with a bowel join, and not a balloon that is later removed. How many folds are placed belongs on the written plan, not in a slogan. Weight loss still depends on what you drink, chew and attend after you fly; a target kilogram is not sold at a fixed fee.

The written plan should identify the endoscopic map, the provider who performs the scope and sutures, and whether sedation or general anaesthesia is planned.

Gastroenterologists and bariatric teams usually consider adults whose BMI sits in a range where an endoscopic fold is offered after review — often when a cut and a staple line are not the first choice — who can commit to diet follow-up, and who accept that sutures can loosen and that a later laparoscopic sleeve is a different quote. If the stomach must be reduced with a stapler, the gastric-sleeve page is the conversation. If a temporary balloon is enough, that is a different sitting. Severe reflux, a previous gastric operation or a large hiatus hernia often changes the map.

The date is often postponed if an untreated ulcer, a clotting risk or a finding on blood tests makes the sitting less safe, or if you cannot pause smoking when asked. Send height, weight, medicines and any recent endoscopy if you have one. Suitability is confirmed in Istanbul before anyone stitches. A laparoscopic sleeve on the same day, a bypass “while we are there”, or a hair transplant in the same week is not assumed.

The written plan should identify the endoscopic sitting, pre-operative examination and named tests, and whether sedation or general anaesthetic is planned. Observation and discharge depend on clinical findings and the ability to keep fluids down.

A laparoscopic sleeve, bypass, balloon, conversion or second ESG sitting is a separate procedure unless independently assessed and named. If anatomy does not allow a safe fold, the endoscopist may stop rather than stitch further; that is a clinical limit.

Allow three to four days in Istanbul for assessment, the endoscopic sitting and the first sip check. The endoscopic team sets observation and discharge after you can keep fluids down; allow more time if advised.

The sitting is through the mouth under sedation or general anaesthetic as named on the paper — not a dental local, and not a hospital night unless that paper keeps you in. Discharge to fly is signed after you keep fluids down — it is not an advertising clock.

The first days are liquids, then texture as written — not a restaurant menu because the folds are new. Walk as asked; that is for clots, not for sport. Desk work is often possible after a day or two if the team agrees. Gym, fizzy drinks and large meals wait for the timetable on your discharge note. Weight change is individual; loose skin later is a different quote. If fullness fades months later, that is a review, not this first sitting.

Red flags after you return — spreading pain, a temperature that persists, vomiting you cannot stop, black stool, or breathlessness — mean a local clinician or A&E first, then your coordinator. Take the discharge note with you.

These notes answer questions people raise before they accept a written treatment plan. They describe what that plan should make clear. They are not a promise that any partner hospital uses a particular technique, device or payment arrangement. With your consent, an enquiry may be shared with an authorised healthcare partner.

Why am I a candidate for this weight-loss procedure, and how do other illnesses change the choice?

Diabetes, sleep apnoea, blood pressure, medicines, earlier treatment and whether long-term follow-up is realistic have to be written for the person. A sleeve, a bypass, a balloon and endoscopic sleeve gastroplasty are not the same assessment. ASMBS describes ESG as an endoscopic option for selected adults who have not lost enough weight with lifestyle measures and medication, who are not ready for surgery, or whose surgical risk is higher. In most of that data the primary range is a BMI of 30–40, and a case outside it needs a separate written reason. ESG does not produce the same weight or metabolic effect as a sleeve or a bypass. A balloon does not take the place of that surgical assessment. Source: ASMBS ESG FAQ.

How do reflux or a hiatal hernia change the choice of sleeve, bypass, ESG or balloon?

Marked reflux or a hiatal hernia does not pass through the same door for a sleeve as it does for a volume-restricting balloon or ESG. A sleeve can increase reflux. In severe reflux, a bypass is discussed separately. Reflux symptoms, and endoscopy if it is needed, should be written before a balloon or ESG is chosen. This is not a promise that every case of reflux will settle with a bypass.

How is endoscopic sleeve gastroplasty different from a surgical sleeve, for me?

ASMBS describes ESG as a volume procedure that preserves the fundus and slows emptying of the stomach. The same weight loss or metabolic effect as a sleeve or a bypass should not be expected. In the MERIT study, mean total body-weight loss at 12 months was 13.6%. Series have reported 15–17.5%. Sutures and anchors can make a later sleeve, bypass or duodenal switch harder and can raise its risk. ESG is not recommended as a procedure on its own, without nutrition support and long follow-up. Source: ASMBS ESG FAQ.

If an urgent problem or a nutritional deficiency appears after I go home, who follows it and how are the costs split?

The NHS asks for lifelong blood tests to look for vitamin deficiency, and for 999 or 111 when emergency symptoms appear. Blood tests, a dietitian visit, another endoscopy and a hospital stay should not be assumed to sit inside the original package after treatment abroad. Travel insurance usually does not cover surgical complications. Sources: NHS recovery after weight-loss surgery, NHS advice on cosmetic surgery abroad.

Are ESG results guaranteed to last?

No weight-loss procedure can guarantee a lasting result. The American Society for Metabolic and Bariatric Surgery describes ESG as having lower expected weight loss than surgical sleeve gastrectomy or gastric bypass, and says evidence about long-term durability is still developing. Outcomes vary, so ESG should be considered within ongoing obesity care. Ask the treating team what nutrition support, follow-up and options for further care are included in the written plan.
