Endoscopic sleeve gastroplasty, often called ESG, uses an endoscope passed through the mouth to place sutures that reduce the stomach’s usable volume and alter its shape. It avoids abdominal incisions, which may mean a different recovery profile from surgery, but “incision-free” does not mean risk-free or suitable for every person seeking weight management.
The decision is best made around the whole programme rather than the procedure label. Weight-related health conditions, previous stomach operations, reflux symptoms, medicines, eating behaviour and willingness to attend follow-up can all affect whether ESG is sensible. A less invasive route has different trade-offs

Assessment may include weight history, metabolic health, nutrition, psychological factors, alcohol use, previous endoscopy or surgery and the ability to follow a post-procedure diet. ESG may be unsuitable or need modification in some anatomical or medical situations. It is reasonable to ask who performs the assessment, what other treatments are discussed, and whether an in-person examination or additional testing could change the initial recommendation.

Follow-up protects the value of the procedure

Seek urgent assessment for severe or worsening abdominal or chest pain, persistent vomiting, fever, bleeding symptoms, fainting or breathing difficulty. Routine discomfort should be discussed early rather than managed through prolonged poor fluid intake.
