A seroma is a collection of clear fluid beneath the tissues lifted during abdominoplasty. Small collections may settle or be observed; persistent or large seromas can require aspiration, repeated visits or another procedure. Prevention focuses on reducing the space in which fluid can collect while limiting tissue trauma and preserving blood supply.
Progressive tension sutures attach the abdominal flap to deeper tissue at multiple points. A 2024 meta-analysis of 24 studies and 750 patients found a lower seroma risk with these sutures than with drains, without a significant pooled difference in haematoma, infection or wound dehiscence. Study methods varied, so the result supports an option rather than proving that drains are obsolete.
Drains and sutures solve the problem differently
A drain removes fluid after it forms and allows the team to monitor output. Progressive tension sutures reduce dead space and distribute tension before fluid accumulates. Surgeons may use either method, combine them or choose another quilting technique. The operation’s extent, simultaneous liposuction, tissue quality and the clinician’s experience influence the choice.
“Drainless” is not the same as risk-free
Avoiding drains can improve comfort and remove the task of measuring output, but it does not guarantee that no seroma will occur. Conversely, having a drain does not prove that the procedure is outdated. Drains bring their own care burden and possible irritation or infection, yet can be useful when output monitoring is important.

Recovery behaviour remains relevant
Garments, activity and drain care should follow the operating team’s protocol. More compression is not automatically better, and reducing movement excessively can create other risks. Sudden swelling, a fluid wave, drainage, increasing pain, fever or redness deserves review. A collection cannot be classified reliably from a photograph alone.
How to use the evidence
Ask for the surgeon’s own definition and follow-up rate for seroma, not only a marketing statement about drains. Discuss how closure technique, haemostasis and aftercare work together, and what the team will do if fluid develops after you return home. A sound plan includes management as well as prevention.
